State Certified Nurse Assistant (CNA) Testing
Ready to take the state exam to become a Certified Nurse Assistant? Occasionally, we have availability for those who are not our students to test.

The state CNA exam is offered in two portions: the knowledge and the skills. The knowledge portion of the exam is completed on a computer with a test proctor. The skills portion is completed in a lab setting where candidates are randomly drawn to demonstrate five (5) skills with an actor and a RN test examiner. Candidates looking to take the state exam can reserve their spot for a test date by submitting payment and a copy of Headmaster's CNA Application Form 1101ND.

Form 1101ND and payment MUST be received in our office by the Friday prior to the test date. For an August 14, 2019 test date, applications and payment must be received by August 9, 2019. No refunds will be issued for registrations without applications submitted by this date.

Payment can be made online with a credit or debit card, or paid directly from your bank account. If you prefer, you may also make a payment over the phone with a credit or debit card at 701-662-1578 during regular business hours. Testing registrations are subject to our cancellation policy.

Form 1101ND can be faxed to 701-665-4670 or emailed to lrsc.trainnd@lrsc.edu. Please indicate which test date you are registering for on the application.

A testing confirmation letter will be emailed to you after we have your complete application.

Testing will be held at the Bergstrom Technical Center, Lake Region State College, 1801 College Dr N, Devils Lake, ND 58301.

Cancellation Policy: Cancellation more than 5 days prior to the test date results in a 25% cancellation fee based on the testing fee. Cancellation 5 days or less to the test date results in no refund.
